Your scale lied to you: visceral fat, body composition, and what really matters

A traditional scale measures only one thing

Take the number you saw this morning. Mentally break it down into four separate piles:

The first pile is your fat mass — all the adipose tissue in your body, both the subcutaneous fat you can see and the visceral fat you cannot.

The second pile is your skeletal muscle mass — the muscles you use to move, which burn calories at rest, and which maintain your metabolic structure.

The third pile is body water — which fluctuates by 1-2 kg over the course of a single day depending on what you’ve eaten, how much you’ve slept, whether you are on your menstrual cycle, or if you ate salt last night.

The fourth pile is bone mass — which changes slowly over the years, but it does change.

A traditional scale sums these four piles and gives you a number. It doesn't tell you how much of each there is. It doesn't differentiate. It can't.

What changes in practice You can lose 2kg of muscle mass and 0kg of fat in a month on a bad diet. The scale will show -2kg. You will think you have made progress. Your basal metabolism will have decreased, your strength diminished, your visceral fat unchanged. This is not progress — it's the opposite.

The 4 components of weight that no one has ever separated for you

Fat mass: total vs. visceral

Total fat is the percentage of adipose tissue relative to body weight. Reference ranges vary by age and sex — for an adult man, 18-25% is normal, for a woman, 25-32%.

But total fat is only half the story. The part that matters clinically is visceral fat — the fat that accumulates in the abdominal cavity, around the liver, pancreas, and intestines. It's not what you see in the mirror. It's what you don't see.

Skeletal muscle mass

This is the tissue that burns calories at rest. One kg of muscle consumes about 13 kcal/day even without doing anything. One kg of fat consumes 4. This is why two people with the same weight can have completely different basal metabolisms.

Muscle mass naturally decreases after age 35 — about 0.5-1% per year if no strength training is done. This process, called sarcopenia, is completely invisible on a traditional scale.

Body water and daily fluctuations

The adult human body is composed of 55-65% water. A single meal rich in sodium can increase water retention by 500-800g overnight. An intense training session can reduce it by 1-1.5kg in a few hours.

Visceral fat: the number your doctor never asks for

Visceral fat is measured on a scale from 1 to 12 (some systems go up to 15 or 20).

  • Levels 1-6: normal
  • Levels 7-9: caution — increasing metabolic risk
  • Level 10 and above: significant cardiovascular and metabolic risk

The critical point: most people don't know their level. Your GP doesn't routinely measure it. Blood tests don't detect it. BMI — the body mass index that everyone quotes — doesn't capture it in any way.

What really happens when "weight doesn't drop"

This is the most frequently asked question. You're following a diet. You're moving. Three weeks later, the scale shows the same number — or worse, a higher number.

There are three real explanations, and only one is "you're not losing fat."

Explanation 1: You are losing fat but gaining muscle.

This exists, and it's caused by metabolic adaptation — the body reduces energy expenditure in response to prolonged caloric restriction. It's not a failure; it's a biological response. It's managed with cyclical variations in caloric intake and maintenance of muscle mass.

Body recomposition: why the scale might go up while you're improving

The most counterintuitive effect of fitness is this: you can weigh exactly the same as six months ago, with your pants two sizes looser.

This happens when you lose fat and gain muscle in parallel — muscle is denser than fat, occupying less volume at the same weight. Two people weighing 70kg can have radically different bodies if one has 15% body fat and the other 30%.

Visceral fat and PCOS
