Body Composition: What It Is, How It's Measured, and Why Weight Alone Isn't Enough

The number on the scale tells you only one thing. Body composition tells you twenty-five—and those twenty-five are far more important for understanding your body's health. This guide explains what body composition is, how it's measured, and why it's worth looking beyond just weight and BMI.

What is Body Composition

Body composition is the percentage distribution of the main compartments that make up the total body weight:

  • Fat Mass (FM): storage and structural lipids. Includes visceral fat (around organs) and subcutaneous fat.
  • Fat-Free Mass (FFM): everything else—muscles, bones, organs, water.
  • Skeletal Muscle Mass (SMM): the muscles we actively use.

Why Weight Alone is Misleading

The problem with weight is that it's blind to composition:

  • A 75 kg person with 35% body fat has twice the cardiovascular risk compared to someone who weighs 75 kg with 18% body fat—despite identical weight.
  • People who exercise regularly often see their weight increase as their body improves—muscles weigh more than fat, and volume decreases even if the scale doesn't show it.
  • Elderly individuals with "normal weight" can have severe sarcopenia—insufficient muscle masked by excess fat (sarcopenic obesity).

Why BMI is Insufficient

The Body Mass Index (BMI = weight kg / height² m) is practical but limited:

  • It doesn't distinguish between fat mass and muscle mass—a bodybuilder can be classified as "obese" with a BMI of 30 despite having 10% body fat.
  • It doesn't detect fat distribution—visceral fat (abdominal) is much more dangerous than subcutaneous fat (buttocks, thighs), but BMI doesn't differentiate them.
  • It varies by ethnicity—Asian populations have metabolic risk even at a BMI of 23.

The WHO itself acknowledges the limitations of BMI as an individual health indicator. Body composition adds the missing dimension.

Normal Muscle Mass Values

Skeletal muscle mass (SMM) is expressed as a percentage of total weight or as an index (SMI = SMM kg / height² m):

Several techniques exist, with varying levels of accuracy and accessibility:

BIA: How Bioelectrical Impedance Analysis Works

Bioelectrical Impedance Analysis (BIA) is the technology behind modern impedance scales. It works as follows:

  1. A very weak electrical current (50 µA, imperceptible) passes through the body from the feet (and hands in 4-electrode models).
