Insulin Resistance: What It Is, How to Recognize It, and How to Reverse It

Insulin resistance is the underlying mechanism of type 2 diabetes, metabolic syndrome, and stubborn belly fat. It can be present for years before blood sugar becomes abnormal — and within that window of time, it is completely reversible. Recognizing it early means having the opportunity to reverse the process.

What is Insulin Resistance?

Normally, when you eat carbohydrates, the pancreas secretes insulin, which "opens the doors" for muscle, fat, and liver cells to absorb glucose from the blood. In insulin resistance, these cells stop responding adequately to insulin — like a lock that no longer responds to its key.

The pancreas compensates by producing more and more insulin (compensatory hyperinsulinemia). As long as it can keep up, blood sugar remains normal — but high insulin levels in the blood cause other problems. When the pancreas becomes exhausted or exceeds its compensatory capacity, blood sugar begins to rise → pre-diabetes → type 2 diabetes.

How to Measure Insulin Resistance: HOMA-IR

The most commonly used indirect test is HOMA-IR (Homeostatic Model Assessment of Insulin Resistance), calculated from a simple fasting blood test:

HOMA-IR = (fasting glucose in mmol/L × fasting insulin in µU/mL) / 22.5

2.0–2.9 Mild-moderate insulin resistance
≥ 3.0 Significant insulin resistance

HOMA-IR requires both fasting glucose and fasting insulin — two blood tests from a single blood draw, prescribed by a doctor.

Signs That May Indicate Insulin Resistance

Insulin resistance rarely has specific symptoms, but there are indirect signs:

  • Visceral abdominal fat: especially if the rest of the body is normal, fat concentrated in the abdomen is a strong sign
  • Acanthosis nigricans: thickening and darkening of the skin in folds (neck, armpits, groin) — a characteristic sign of hyperinsulinemia
  • Intense fatigue after meals high in carbohydrates
  • Intense cravings for sweets or carbohydrates in the afternoon
  • Difficulty losing weight despite diet and exercise
  • High triglycerides and low HDL in blood tests
  • Hypertension (hyperinsulinemia increases renal sodium reabsorption)
  • Polycystic ovary syndrome in women (PCOS is strongly associated with IR)

Connection with Metabolic Syndrome

Metabolic syndrome is a cluster of risk factors that often occur together, all related to insulin resistance. Diagnosis requires 3 of the following 5 criteria:

Around 25–30% of Italian adults have metabolic syndrome (CUORE ISS Project 2024) — many without knowing it.

7 Evidence-Based Strategies to Reverse Insulin Resistance

1. Physical activity (the most powerful)

Exercise is the single most effective intervention for improving insulin sensitivity, because contracting muscle absorbs glucose through insulin-independent pathways (GLUT-4 pathway). Effects are visible within 1–2 weeks of regular training.

  • Aerobic: 150 min/week of moderate intensity reduces HOMA-IR by 25–35%
  • Resistance: 2–3 weight training sessions per week further improve insulin sensitivity and build muscle (the primary "reservoir" for glucose)

2. Reduction of visceral fat

Visceral fat produces cytokines that directly interfere with insulin receptors. Losing it — even just 5–10% of body weight — significantly improves insulin sensitivity.

3. Low glycemic index / low-carb diet

Reducing high-GI carbohydrates (or reducing total carbohydrates) lowers compensatory hyperinsulinemia, giving the pancreas time to "rest." Low-carb diets (<130 g/day) improve HOMA-IR by 30–50% in 3 months in pre-diabetic subjects.

4. Intermittent fasting / eating windows

Periods of fasting (16h/24h or 2 days a week) improve insulin sensitivity and reduce hyperinsulinemia. Effective even without calorie reduction — the effect is mediated by reducing the time spent with high insulin.

5. Quality sleep

A single night of 4 hours of sleep reduces insulin sensitivity by 25% the next day. Chronic insufficient sleep (<6h for 2 weeks) produces metabolic deterioration equivalent to 6 months of a high-calorie diet.

6. Reduction of chronic stress

Elevated cortisol reduces the expression of insulin receptors in muscle and promotes hepatic glucose production. Stress reduction techniques (meditation, yoga, breathing) improve insulin sensitivity in 8–12 weeks.

7. Metformin (under medical supervision)

For individuals with pre-diabetes and elevated HOMA-IR, a doctor may prescribe low-dose metformin — it reduces hepatic glucose production and improves peripheral insulin sensitivity. It's not necessary for everyone, but it is one of the most effective and safe pharmacological options for diabetes prevention.

How to Monitor Improvement

Improvement in insulin resistance is monitored by:

  • HOMA-IR (blood test every 3–6 months)
  • Fasting glucose — drops towards optimal values (<90 mg/dL)
  • Body composition — reduction of visceral fat on BIA scale
  • Waist circumference — measurable reduction in 4–8 weeks of correct lifestyle
